Job Title: Nurse Practitioner (NP) – Wound Care
Location: Skilled Nursing Facility, Manhattan, NY
Employment Type: Full-Time, Contract
Schedule: Monday – Friday, business hours (with occasional weekend rounds if needed)
Compensation: Competitive salary based on experience + benefits
Position Summary:
We are seeking a compassionate and experienced Nurse Practitioner (NP) with a strong background in wound care to join our clinical team at a skilled nursing facility. The ideal candidate will have expertise in assessing, diagnosing, and treating complex wounds, including pressure injuries, diabetic ulcers, surgical wounds, and other skin conditions commonly found in the SNF population. This role is critical in promoting optimal healing outcomes and preventing complications through high-quality, patient-centered care.
Key Responsibilities:
Conduct comprehensive wound assessments and develop individualized treatment plans
Perform debridement, dressing changes, and other advanced wound care procedures as needed
Collaborate closely with nursing staff, physicians, therapists, and the interdisciplinary team to ensure coordinated care
Monitor wound progression and adjust treatment plans based on healing trajectory
Provide education to patients, families, and staff on wound prevention and care techniques
Document all care and procedures in accordance with facility policies and state/federal regulations
Participate in quality assurance and infection control initiatives related to wound management
Assist with staff training and clinical education related to skin and wound care protocols
Qualifications:
Current and unrestricted Nurse Practitioner license
Certification as a Wound Care Specialist (e.g., WCC, CWCN, or similar) highly preferred
Minimum of 1–2 years’ experience in wound care, ideally in a SNF or post-acute setting
Strong clinical assessment and procedural skills
Proficient in EMR documentation
Excellent interpersonal and communication skills
Ability to work independently and collaboratively within a multidisciplinary team
