The Home Health Registered Nurse (RN) provides skilled nursing care to patients in their homes, ensuring high-quality and compassionate care. This role involves patient assessments, treatment planning, medication management, and education while working collaboratively with interdisciplinary teams to achieve the best patient outcomes.

Essential Responsibilities:

• Conduct comprehensive in-home patient assessments and develop individualized care plans.
• Administer medications, wound care, IV therapy, and other nursing interventions as prescribed.
• Educate patients and families on disease management, treatment plans, and self-care techniques to promote independence.
• Monitor patient progress, recognize changes in condition, and adjust care plans accordingly.
• Collaborate with physicians, physical therapists, occupational therapists, and other healthcare professionals to ensure coordinated care.
• Maintain accurate and timely documentation of patient care following state, federal, and agency regulations.
• Ensure compliance with OASIS documentation, Medicare, and insurance guidelines (if applicable).
• Provide emotional support and guidance to patients and their families.
• Adhere to infection control and patient safety protocols.
• Travel to patient homes within the assigned service area and manage multiple patient visits per day.

Qualifications:

• Active Registered Nurse (RN) license in California.
• Minimum 1 year of nursing experience (home health experience preferred but not required).
• Knowledge of OASIS documentation and Medicare regulations (preferred).
• Strong clinical assessment, critical thinking, and communication skills.
• Ability to work independently and manage time effectively.
• Proficiency in electronic medical records (EMR) systems.
• Reliable transportation, a valid driver’s license, and auto insurance.
