Pathway Hospice, the leading locally owned hospice provider in DFW, is seeking an experienced RN to join our clinical team as an Admissions RN. The ideal candidate will provide exceptional service to Pathway’s patients, families and partners. While our office is in Richardson, TX, we service patients throughout the DFW Metroplex.

Responsibilities of this position may include the following:

• Collaborate with the patient, caregivers, clinical manager, case managers, and physician(s) to complete the patient care planning at the onset of care
• Serve as the primary staff member leading the transition of a patient's care into our hospice setting in a timely fashion
• Provide nursing assessment, initial plan of care and intervention(s), which optimize patient and family directed comfort and quality of life
• Provide daily GIP visit services, if applicable
• Serve as a patient and family advocate to ensure delivery of the highest quality of care, providing education to patients, families, and caregivers
• Complete documentation in a timely manner
• Participate in patient care meetings as assigned
• Communicate effectively with coworkers and patients/family
