Clinicare Pharmacy is accepting applications for a Clinical Pharmacist with an ambulatory care/comprehensive disease and medication management focus. This position will be reporting to the Director of Pharmacy, while working in the Advanced Practice Pharmacy Care Center (Clinicare APP) in providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients.

Responsibilities will include providing initial and follow up patient consultations and chronic disease and medication therapy management, and proper documentation in accordance with the pharmacy’s policies and procedures and standards of advanced practice pharmacy; ensuring accurate and safe therapy, adherence monitoring, assisting with the health plans formulary and the prior authorization process. The clinical staff pharmacist will also complete tasks as assigned by the Director of Pharmacy and will supervise ancillary staff (technicians, interns) and provide guidance as necessary.

The applicant must have a degree from an accredited (ACPE) school of pharmacy and be a registered pharmacist in the state of CA.

Minimum Education: PharmD degree from an ACPE accredited School of Pharmacy with 3 years of clinical pharmacy or ambulatory care pharmacy practice within community practice setting with direct patient care services, OR completion of PGY-1 program in community or ambulatory care pharmacy practice from ASHP-accredited Residency Program.

Minimum Field of Expertise: Pharmacy/California State License.

Preferred Experience: Work experience in a community pharmacy practice with direct patient care/clinical services.

In this position, the clinical pharmacist will:

  • Provide and assure safe and effective medication use
  • Attend and successfully complete general and department orientation, in-service programs and the annual key competencies and mandatory update requirements
  • Demonstrate a comprehensive clinical pharmacy knowledge base
  • Perform interactive, patient-centered, in-person and/or telephonic/video comprehensive medication reviews (CMRs) with patients.
  • Evaluate medication profiles to determine if changes are warranted to optimize the patient’s drug therapy and minimize the potential for adverse drug reactions.
  • Use motivational interviewing skills to improve a patient's adherence to medication during telephone calls/telehealth/in-person appointments.
  • Recommend alternative medications, alternative dosing, or other modifications (including discontinuation of medications) as appropriate.
  • Review therapy plans with physicians as needed.
  • Review patient care reports, clinical notes and other medical records as needed for medication action plan development, targeted clinical outreach programs and quality assurance purposes.
  • Implement medication optimization processes that meet the needs of managed care organizations and their members.
  • Work in a multidisciplinary team environment to develop and enhance MTM program and various software and EHR systems used to manage the program.
  • Participate in other non-dispensing activities (i.e. immunizations, clinical programs, population health initiatives, etc).
  • Work directly with patients, pharmacists, physicians, and other health care practitioners.
  • Participate in external and internal interdisciplinary care team meetings and produces MTM related documentation to be shared with appropriate treating providers.
  • Demonstrate willingness & ability to practice in areas within their scope of practice as necessary (i.e. working in the outpatient pharmacy department and practice patient-centered dispensing)
  • Implement and maintain established pharmacy and APP department policies, procedures, and objectives as well as improving organization performance program, safety, environmental, and infection control policies
  • Coordinate and/or provide comprehensive clinical pharmacy services to patients
  • Coordinate ongoing quality assessments and improvements of clinical pharmacy activities in collaboration with physicians and implement necessary action as appropriate
  • Effectively communicate verbally and/or in writing clinical pharmacy functions to the interdisciplinary team
  • Serve as a drug information resource person to physicians, patients and other health care staff
  • Participate actively in educational activities and programs for the interdisciplinary team and in department staff meetings
  • Maintain competency and enhance professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops
  • Serve as a preceptor to pharmacy residents and students when applicable
  • Maintain required applicable licensure, certification and education.
  • Support Clinicare Pharmacy and Advanced Practice Pharmacy Care Center as necessary with any tasks required to deliver excellent personalized patient care and perform all other duties as assigned.
