JOB DESCRIPTION: HOME HEALTH OCCUPATIONAL THERAPIST
Job Summary:We currently have an opening for a PRN Occupational Therapist in a growing Home Health company. Suncrest Home Health-Southside pledges to be person centered, quality focused and community minded. You will provide professional support for home care following established standards and practices. Occupational therapy at home works on strategies with the patient to safely perform needed daily activities or “occupations. Physical therapy helps improve patients’ strength, balance and mobility by treating the physical source of the patient’s issue. They help patients improve, regain, and develop the skills needed for day-to-day life and work. They may provide long-term patient care and acute patient care.

Duties/Responsibilities:- Reviewing patients' medical histories and asking them questions about the challenges they face
- Evaluating patients' conditions by observing them as they execute tasks and communicating with them about how they feel.
- Identifying specific goals and developing subsequent treatment or recovery plans
- Demonstrating exercises and physiotherapy techniques
- Recommending therapy equipment based on patients' requirements and teaching them how to use or operate the equipment.
- Visits by an occupational therapist are to assess and reassess a beneficiary's rehabilitation needs and potential, or to develop and/or implement an occupational therapy program, are covered when reasonable and necessary because of the beneficiary's condition.
- Recording patients' activities and progress
- Devise detailed treatment and care plans for each of the patients.
- Communicate and coordinate with patients’ physicians to ensure a consistent level of care.
- Visit patients in their homes and at other facilities on a limited basis.
- Exercise independent judgment based on experience to implement effective treatment combinations.
- Evaluate and report on patients’ progress.
- Assist administrative staff with billing and reporting inquiries.
- Comply with federal and state physical therapy regulations.
- Recording patients’ progress and modifying the plan of care if needed
- Educating patients and family members about the recovery process
- Perform other duties as assigned.
- Excellent written communication skills and interpersonal skills
- OT Therapy visits in Home Health Environment.
- Able to help with a wide variety of other issues including social, mental, cognitive and physical challenges as an OT in Home Health environment.
- Excellent organizational skills and attention to detail.
- Excellent time management skills with a proven ability to meet deadlines.
- Strong analytical and problem-solving skills.
- Proficient with Microsoft Office Suite or related software.
- Experience working effectively in a diverse environment is preferred.
- Responsible for all aspects of occupational therapy service delivery provided under a physician approved plan of care.
- Ensures the plan of care addresses all problems identified in the assessment or documents rationale if identified problems are not addressed in the plan of care.
- Accountable for the safety and effectiveness of the occupational therapy service delivery process.
- Completes documentation timely and accurately.
- Supervises home health aide when RN is not active in the case.
- Other duties as assigned.
- Current license to practice occupational therapy specific to the state you are assigned to work.
- Graduate of an occupational therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOTE
- Has successfully completed, the entry-level certification examination for occupational therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).
- One-year of related occupational therapy experience to ensure knowledge and skills are sufficient to safely provide occupational therapy services to patients.
- Current CPR certification.
Physical Requirements:
- Frequent exposure to communicable diseases and other conditions common in a clinic environment.
- Prolonged periods of sitting and working at a computer.
- Occasionally you may lift and carry boxes of material and equipment weighing 10-15 pounds.
