Job Description
Nexus Health Resources seeks an on-site, dedicated, and compassionate individual to join our team as a Care Coordinator and Surveyor. In this role, you will be responsible for conducting phone surveys in both English and Spanish. Additionally, you will conduct check-in phone calls in both English and Spanish with patients who have recently been discharged from hospitals or nursing homes, ensuring their well-being and aiding as needed. Communication with patient care team members to relay patient concerns is also crucial to this position. The successful candidate will report to the Nexus Health Call Center Manager.
Responsibilities:
- Conduct surveys to collect patient feedback
- Conduct supportive check-in calls for recently discharged patients, addressing concerns and providing necessary assistance
- Effectively communicating with the patient's care team, promptly relaying concerns and feedback
- Maintain accurate documentation of survey responses, patient interactions, and communications among the care team
- Collaborate with colleagues and the Call Center Manager for streamlined patient care
- Proactively identify and address issues during patient interactions, escalating when necessary
- Adhere to all protocols and guidelines for surveys, patient interactions, and compliance with healthcare regulations
- Participate in ongoing training for industry best practices and contribute to continuous improvement efforts
- Provide exceptional survey and call experience, demonstrating empathy and active listening
Qualifications:
- High School diploma
- Fluent in English (reading, speaking, and writing)
- Fluent in Spanish is a plus (reading and speaking)
- Strong verbal and written communication skills
- Sunny/positive disposition
- Strong phone etiquette
- Proficient in using the company software (training provided)
- Demonstrates a strong sense of responsibility/accountability
- Sets high expectations for quality
