We are seeking a highly organized and compassionate LVN with strong administrative and clinical experience to join our Home Health team. This  role is perfect for a nurse who not only understands the inner workings of clinical care but can also be a point of contact working with clinical staff, admin team and leadership.

The ideal candidate has thorough knowledge of Home Healthcare  operations, Medicare documentation requirements, and is comfortable supporting the clinical and administrative flow of the agency. This role is about 50% patient care and 50% office depending on circumstances.

Responsibilities

• Provide administrative support to hospice leadership and clinical teams
• Coordinate patient schedules, documentation, and staff communication
• Ensure compliance with hospice documentation standards and CMS regulations
• Assist with meeting prep, physician orders, and plan of care updates
• Perform  patient visits as needed (within LVN scope of practice)
• Help maintain accurate EMR records and assist with QA follow-ups
• Support survey readiness and organizational tasks
• Qualifications:
• LVN license in good standing (California preferred)
• Previous experience in Home Healthcare care Minimum of 1 year required
• Strong understanding of Home Halthcare documentation, care planning, and compliance
• Excellent communication, time management, and organizational skills. Able to work independently and handle shifting priorities
• EMR proficiency, CPR, Driver's license, Auto Insurance, Microsoft  365 and teams.
