Job Description
Joseph’s House is a residential respite care home in NW Washington, DC that serves primarily previously homeless men and women with advanced HIV disease and/or terminal cancer. For most of these residents, some combination of mental illness, stigma, isolation, substance abuse, poverty, and/or domestic violence have prevented them from successfully managing their HIV disease and cancer.
Once medically stabilized, many Joseph’s House residents transition to post-residential case management. These services include medication management, housing, counseling, life skills training, advocacy, transportation, and community activities.
Job Summary:
The Director of Clinical Services at Joseph’s House is a multidisciplinary team member, including a registered nurse, clinical coordinator, executive director, and staff, who provides comprehensive intake, assessment, discharge planning, and case management services to all Joseph’s House Community members. The Director of Clinical Services works directly with the executive director, nurses, a social worker, a non-medical case management team, staff, residents, community friends, and community partners. The Director of Clinical Services supports the Executive Director in managing and administering the daily operations of Joseph’s House to ensure that quality of care and financial and legal standards are met in alignment with the expectations of residents, government authorities, and private foundations.
Responsibilities:

- Ensure high-quality, person-centered care is provided in alignment with medical and behavioral health standards
- Supervise clinical staff, including nurses, aides, and social workers, ensuring proper documentation and adherence to care plans
- Monitor resident health status, review assessments, and oversee the development and implementation of individualized service plan (ISPs).
Program Management and Service Delivery
● Provide information, advice, and counsel to the Executive Director and management in Joseph House services' planning,
development, and operation

● Recruit, train, and supervise staff, ensuring adherence to best practices

● Assist with grant applications and funding efforts while managing monthly program data entry and reporting

● Build rapport and establish trusting, respectful relationships with clients and families
● Participate in evaluating prospective, new, and existing clients based on their needs, barriers, and goals
● Assist in developing an individualized discharge plan for every resident
● Develop and regularly update a Service Plan for every community member
● Address client concerns and goals in real-time using practical communication skills with the client and nurse case management

● Support the Executive Director in developing and implementing strategic initiatives
● Collaborate with staff and external stakeholders to identify additional opportunities for programmatic innovation and expansion that

effective processes/plans to move the organization forward.
Compensation
● $88,000 - $100,000 annual salary

Education and Experience
● At least 5 years of case management and community outreach experience
● A background in nursing and a current DC RN or LPN license are required
● A minimum of 2 years working with vulnerable populations, including homelessness, substance use, mental illness, and HIV/AIDS
● Experience in fundraising, as well as participating in and speaking at meetings and panel discussions
● Previous experience in management is a plus.

We are seeking candidates who are local to the DC Metro area. This hybrid position requires a few days in the office each week with some flexibility in scheduling.
