Qualifications:

Must have a Minimum of high school diploma or equivalent with one (2) year of work or life experience. Current CPR, AED and First Aid Certification as well as Nurse Safe Training and Fire Training; or the ability to obtain certs within 3 months of employment. Excellent interpersonal and communications skill, ability to demonstrate respect for individuals with I/DD and their families, and ability to lift 25 pounds. Experience working with patients with disabilities to include mental/behavioral health preferred.

 

Purpose/Summary:

To provide supervision, support, and direction to individuals with disabilities (mental and behavioral) and at-risk patients, while maintaining a safe, and caring environment in which clients are present. The case manager will assist with daily living activities and provide behavioral support, medication management and crisis intervention in a group home setting.

 

Essential Functions and Responsibilities:

  • Provides support to clients in the Activities of Daily Living (ADL) and personal care needs
  • Provides direct supervision in a group home setting
  • Assists with maintenance and upkeep of the group home with the assistance of resident clients
  • Implement daily activity schedule, ensure all activities are conducted on time and supervised.
  • Provides positive behavioral supports daily.
  • Complete rounds of unit every 30 minutes and ensure that all clients are accounted for
  • Provides input into assessments, conferences and treatment planning
  • Organizes and supervises recreational and social activities
  • Collects and records data and completes all required documentation.

 

 

  • Conduct multiple daily checks of clients’ rooms and all other areas to ensure cleanliness and order. A record of the daily checks is to be recorded and submitted to the management.
  • Follows policies and procedures for the monitoring of clients’ medications
  • Intervenes appropriately with clients to prevent, avert or manage crisis situations
  • Responsible for ensuring a safe, therapeutic environment and adhering to all safety standards, Federal and State regulations, and corporate policy.
  • Attend monthly group supervision with Clinical Supervisor as scheduled.
  • Comply and abide with the agency’s Code of Ethics, as well as program’s policies and procedures.
  • Uphold organizational mission, values, vision and support the integrated process of care while exhibiting integrity and ethics in all endeavors.
  • Interact with clients, staff, and client family members in an appropriate and professional manner.
  • Perform other tasks as necessary or as assigned by a member of New Harmony’s Administration.

 

 

 

Additional Requirements:

  • Know the names of all clients on the unit and their room assignments.
  • Punch in/out through Paychex system.
  • Notify immediate supervisor, directly within one hour of normal starting time if running late, and four hours prior to shift stating the specific reason for absence.
  • Know and enforce New Harmony’s Policies and Procedures.
  • Ensure Policy and Procedure for Critical Incident Reporting is followed when indicated.
  • Respond to emergencies and crises involving patients or staff.
  • Ensures compliance with all required state and federally mandated background and exclusion checks including but not limited to criminal history background investigation (fingerprinting), Central Registry, Child Abuse Record Information (CARI), drug testing, and motor vehicle checks.
  • Completion of fire drills and other Health and Safety issues are complied with, and forms are completed accordingly.

Ability to:

  • Produce written documents in English with clearly organized thoughts using proper sentence construction, punctuation and grammar.
