DEPARTMENT: Bereavement Department/Hope Counseling
REPORTS TO: Director of Strategic Initiatives and Special Projects
SUMMARY: Under the supervision of the Director of Strategic Initiatives and Special Projects, the Mental Health Clinician is responsible for helping people manage and overcome mental and emotional issues, providing support for clients who are experiencing impending losses or support for after a loss has occurred, and the delivery of bereavement services, which may include support group facilitation, presentations, and events.
RESPONSIBILITIES:
- Conducts screenings for adult and children/teens. Provides, coordinates, documents, evaluates, and appropriately refers clients to groups and/or community/professional referrals for individuals/family.
- Timely provides mental health assessments, diagnosing, counseling, grief support, anticipatory grief support, referrals, and intervention services. Develops care plans and documents care to adults, groups, communities and children/teens; in homes, schools, facilities and groups.
- Facilitates or supervises support group(s), including evaluation, documentation and support for group members/facilitators and co-facilitators.
- Maintains documentation of client contacts, consultations, and referrals, accurately and timely in accordance with organizational policies.
- Collects data sufficient to meet both internal and external organizational needs and prepares monthly statistics report for internal and external stakeholders accurately and timely.
- Follows all policies and procedures including, but not limited to; safety, infection control, privacy, and confidentiality.
- Performs other duties, as assigned by supervisor, such as bereavement calls, speaking engagements, assisting at department events such as Coping with the Holidays Workshop, Family Tree Ceremonies, etc. May also work in other areas to cover absences or relief to equalize peak work periods or otherwise meet the needs of the organization.
SUCCESS FACTORS:
- Ability to adhere to and ensure compliance with policies and procedures, and federal and state hospice regulations as it pertains to the hospice and counseling programs.
- Ability to collect, organizes, and analyzes data at a level sufficient to document regulatory compliance.
- Ability to communicate on a professional level in both written and verbal formats.
- Ability to develop and maintain cooperative, constructive relationships with coworkers and clients.
- Ability to handle confidential information and disseminate appropriate information to coworkers and other departments appropriately.
- Ability to identify and resolve routine and non-routine problems.
- Ability to be self-motivated and to work independently.
- Ability to meet deadlines and effectively execute multiple projects and tasks simultaneously.
QUALIFICATIONS:
- Master’s degree in counseling, social work or related field is required.
- Licensed Clinical Social Worker or Licensed Marriage Family Therapist CA professional license.
- Extensive knowledge of grief and bereavement processes is required.
- One-year of experience in a hospice or social services setting preferred.
- One-year in a position that required coordination and/or administration of services.
- Intermediate to advanced computer data entry and database maintenance skills required.
- Valid California Driver’s License, reliable transportation, and current auto insurance.
- Bilingual Spanish preferred.
WORK ENVIRONMENT: This position works in an office environment using a computer and general office equipment.
WORK SCHEDULE: This position is regular full-time required to work a regularly scheduled 40-hour workweek, with overtime as needed to meet the needs of the organization. Part-time (20 – 31 hours per week) or temporary positions may be occasionally utilized, if justified by department needs.
