OPTA Home Healthcare, a leading provider of compassionate, high-quality home healthcare services in the Denver metropolitan area, is seeking a dedicated Licensed Practical Nurse (LPN) to join our team. Job Duties/Responsibilities

  1. Provide skilled nursing care to patients in their homes, following the established care plan and physician's orders.

  2. Perform patient assessments, monitoring vital signs, and providing appropriate interventions as needed.

  3. Administer medications, injections, and treatments, maintaining accurate documentation of patient responses.

  4. Assist with activities of daily living (ADLs) and provide guidance to patients and caregivers on self-care and disease management.

  5. Collaborate with the interdisciplinary care team, including RNs, physical therapists, and physicians, to ensure comprehensive patient care.

  6. Document patient progress, changes in condition, and any other relevant information using electronic health records.

  7. Maintain open communication with patients, caregivers, and healthcare professionals to ensure optimal patient outcomes.

  8. Adhere to all applicable regulations, policies, and procedures to ensure compliance with state and federal guidelines.

Qualifications

  1. Current LPN license in the state of Colorado, in good standing.

  2. Minimum 1-2 years of experience as an LPN, preferably within a home healthcare or related setting.

  3. Strong clinical skills, including patient assessment, medication administration, and wound care.

  4. Excellent communication and interpersonal skills, with the ability to work effectively with patients, caregivers, and healthcare professionals.

  5. Familiarity with electronic health records and documentation procedures.

  6. Current Basic Life Support (BLS) certification.

  7. Valid driver's license and reliable transportation for travel to patient homes within the Denver metropolitan area.
