United Way of New York City (UWNYC) is partnering with SOMOS Healthcare Providers, Inc. to deliver capacity-building and technical assistance to approximately 50 community-based organizations (CBOs) in the Bronx County Social Care Network (SCN). Part of New York State’s Medicaid Section 1115 waiver initiative, this effort aims to ensure Medicaid recipients are screened for health-related social needs (HRSNs), connected to services, and supported in achieving better health outcomes.

As implementation partner, UWNYC will coordinate support for CBOs by developing a capacity-building framework, delivering targeted trainings and technical assistance, aligning with SOMOS on data tracking, and promoting long-term sustainability of effective CBO participation.

Position Overview:

Reporting to the Associate Vice President, Health Equity, the Director, 1115 Medicaid Waiver Initiative will oversee strategy and implementation of UWNYC’s capacity-building work with SOMOS. The Director will ensure timely execution of all project components, working closely with SOMOS staff, UWNYC teams, and CBO partners to strengthen screening and navigation capacity. This position is grant-funded through May 1, 2026.

Primary Responsibilities:

Program design and implementation:

  • Oversee coordination and implementation of UWNYC’s capacity building program for CBOs in the SOMOS Care Network, ensuring tasks are completed on time and aligned with project goals.
  • Contribute to the design of and lead implementation of a capacity building framework that articulates the central goals, structure, and support offerings that will be available to CBOs based on existing capacity.
  • Assess CBOs against UWNYC’s capacity building framework and identify relevant supports and technical assistance opportunities that align with needs of CBOs.
  • Design and lead the implementation process for trainings, learning experiences, instructional guidance, and tools and templates that support CBOs to effectively screen eligible Medicaid recipients and navigate them to services.
  • Oversee data collection and reporting policies in accordance with State law and aligned with SOMOS’s performance measurement approach.
  • Engage the Health Equity Unit, Food Benefits Access team, and other relevant teams across UWNYC to identify key issues, gaps, strategic needs, and priorities, recommend solutions to address needs, and work with teams to implement solutions.
  • Supervise Capacity Building Managers (4) and Project Associate (1) in the development, execution, and assessment of tailored capacity-building plans for community-based organizations (CBOs), ensuring alignment with project goals and consistent progress toward key deliverables.
  • Collaborate with the Associate Vice President of Health Equity to evaluate SOMOS program design and effectiveness, supporting real-time learning and program adaptations grounded in community feedback and program data.
  • Identify and amplify lessons learned for UWNYC’s broader health equity portfolio and for SOMOS.
  • Complete other special projects as assigned.

Partnership management:

  • Lead day-to-day management of UWNYC’s partnership with SOMOS, working closely with SOMOS program leads and staff to ensure alignment around progress towards meeting objectives.
  • Represent UWNYC in meetings with SOMOS and external stakeholders; prepare and present project updates; develop and deliver meeting agendas and associated materials.
  • Support Capacity Building Managers to build strong, trusting relationships with CBOs, intervening to support and work directly with CBOs when necessary.
  • Help manage relationships and secure due diligence documents for CBOs to ensure contract execution and grant(s) compliance, in collaboration with the UWNYC Finance and Legal/Administration staff as needed.

  • Contribute to and maintain thorough project planning practices to ensure all programmatic work, including evaluation, is planned, documented, and tracked with fidelity—both within the team and with key internal and external stakeholders as projects demand. Skills, Knowledge, and Experience:

  • A Bachelor’s degree in sociology, politics, public administration or related social sciences, with a minimum 4 years of related work experience or Master’s degree (preferred) in social work, public administration, public health, sociology or related fields with a minimum of 2 years work experience.
  • In-depth knowledge of health equity, social determinants of health, and social care networks in New York City and/or State. Familiarity with Medicaid policies, billing practices, and 1115 waiver strongly preferred.
  • Proven leadership and management experience, including supervising teams to ensure the effective delivery of project goals, with expertise in capacity building, program management, and organizational development.
  • Demonstrated ability to build and maintain partnerships with diverse stakeholders, including community-based organizations, government entities, and health system partners.
  • Exceptional communication skills, with the ability to present information clearly and concisely to various audiences, including community partners, senior leadership, and external stakeholders.
  • Strong project management and organizational skills, with the ability to manage multiple tasks, set priorities, and meet deadlines in a dynamic, fast-paced environment.
  • Ability to synthesize complex data and feedback to inform strategic decision-making, program development, and continuous improvement.
  • Proficiency in Microsoft Office (Excel, Word, PowerPoint) and Google Sheets
  • Must be willing to regularly travel within the 5 boroughs of NYC.
  • Bilingual in English and Spanish strongly preferred.

  • The final determination of salary, within this range, will depend on the candidate’s education, skills and experience required for the position.
