Physician Consultant Opportunity – Long-Term Care Setting (Part-Time/Per Diem)

Are you a compassionate, detail-oriented physician looking for a flexible opportunity to impact patient care without the demands of daily prescribing or order entry?

We are seeking a Physician Consultant to join our collaborative care team providing oversight and guidance for patients in long-term care settings. These patients are primarily managed by experienced nurse practitioners and physician assistants. Your role is to serve as a clinical consultant—offering your expertise to enhance the quality and consistency of care.

Key Responsibilities:

• Perform physician-level assessments (in person or via telehealth)
• Review medical records, medications, and active treatment plans
• Identify gaps in care and recommend pathways or guidelines for chronic disease management
• Provide non-prescriptive guidance to improve clinical outcomes and support mid-level providers
• No prescribing or order placement required

Ideal Candidate:

• Board-certified or board-eligible physician (Family Medicine, Internal Medicine, or Geriatrics preferred)
• Strong interest in consultative medicine and long-term care populations
• Excellent communication skills and a collaborative mindset
• Experience with quality metrics, chronic care, or geriatrics is a plus
