Pathways to Housing DC (Pathways DC) is based in Washington, DC and serves adults recovering from/experiencing homelessness in the District and Montgomery County, MD. We are an innovative and nationally recognized nonprofit committed to ending homelessness for individual with complex health challenges. Pathways DC is one of the originators of the "Housing First" model to end homelessness, in which services are provided without conditions and pre-housing requirements such as curfews, mandated sobriety or compliance with medication. We believe housing is healthcare and that housing is a basic human right. Pathways provides an array of services including housing, street outreach, case management, drop-in services, and behavioral health support. Our model is based on client choice and the idea that the people we serve are the architects of their recovery!

The Licensed Practical Nurse (LPN) is an integral part of the inter-disciplinary Assertive Community Treatment (ACT) team, providing primary health care to adults with serious mental illness in order to promote wellness, recovery and rehabilitation. The day-to-day work of the LPN is supported by the ACT Team’s RN. Responsibilities are implemented within the framework of agency policy, professional ethics, legal scope of practice and regulatory guidelines. Services are provided at ACT team sites and in the community.
RESPONSIBILITIES:
- Conduct home visits to assist consumers with medication management and health education;
- Collaborates with other health care providers and makes appropriate referrals;
- Teach health promotion to consumers and staff;
- Advocate for consumers in accessing and navigating the heath care system;
- Deliver medication, teach medication adherence, take vitals in home settings;
- Work closely with the Team and consumer in establishing and following the optimal treatment/service plan;
- Participate in check-in meeting and case reviews, and consults with team members as needed;
- Collaborate with staff to coordinate prescriptions and orders, and to oversee the overall health of consumer;
- Engage qualified consumers in medical services, both on site and in the community;
- Participate in facilitating appropriate hospitalizations (medical and psychiatric), crisis interventions and referrals;
- Communicate with hospital staff to ensure optimal treatment and discharge planning;
- Perform other related work as assigned.
QUALIFICATIONS:
- Patience, creativity, flexibility, compassion and sensitivity to persons with disabilities and other minority populations;
- Licensed as Licensed Practical Nurse (LPN) in Washington DC;
- Certification by the American Nurses Credentialing Center
- Experience or training in community preferred;
- Experience with dually diagnosed population preferred;
- Excellent written and verbal communication skills
