Summary: The Program Specialist coordinates and ensures that all required needs and services are available for individuals in the program. This position acts as an advocate for the individuals, and a position of support for all program/specialized services.
Supervisory Responsibility: No direct supervisory duties, however, responsible as part of the management team to address any health and safety issue to appropriate management personnel if any areas of concern encountered while performing their required duties.
Essential Functions:
- Complete quarterly reports, annual assessments, IP outcomes, SEEP plans and required paperwork for the 6400 regulations and PA 6100 regulations for all individuals on caseload.
- Coordinate and attend IP monitoring for all individuals on caseload.
- Coordinate and attend all IP meetings, SIS assessments, and any other outside reviews.
- Participate in medical appointments, as needed, to ensure services are requested if needed and that information is relayed appropriately.
- Review all individuals’ IP documents and notify the Supports Coordinator of any errors or omissions.
- Enter/close out EIM reports as needed and in a timely manner.
- Coordinate external therapies coming into the home i.e. PT, OT, speech, sensory, and any other required support.
- Complete intake paperwork, initial assessment, initial IP and initial staff training.
- Conduct initial training and annual training for staff on Outcomes, SEEP Plans, Dysphagia Protocol’s, Seizure Protocol’s, Supervision Needs, ISP’s, Lifetime medical history, Health Promotion Plans and Residential Assessments.
- Attend all staff meetings and training sessions as required.
- Prepare and maintain all required licensing files throughout the year for each individual.
- Role model for all direct care staff in all outcomes, SEEP plans, and on basic functional job skills in the best interests of the individuals i.e. Positive approaches, environmental awareness, etc.
- Work hand in hand with Direct Support Professionals, Agency nurse and Management Team if a crisis arises in the home.
- Present individuals, staff and Agency in a positive manner to neighbors, medical personnel, families and the public at large.
- Coordinate vacations to destinations of individual’s preference, if desired.
- Fill in at assigned sites on an emergency basis.
- Respond to emergency calls as needed.
- Participate in the on-call rotation of the agency.
- Certifications/proficiency in training as required by the role.
- All other tasks as deemed necessary by the Agency management.
Minimum Qualifications:
- Bachelor’s degree in human services related field and at least 2 years related work experience. Associates Degree in a related field and at least 4 years related work experience.
- Ability to demonstrate experience in training abilities.
- Valid driver’s license.
Knowledge, Skills, Abilities and Core Competencies
- Knowledge of 6400 regulations and PA 6100 regulations.
- CPR, First Aid certification.
- Medication administration certification and certification as an ODP Medication Administration Trainer.
- ODP Certified Investigator training completed.
- Good communication and writing skills.

- Regularly required to sit, stand and walk for periods of time, occasionally climb and descend steps, and frequently talk and hear. The employee is frequently required to use hands and fingers to handle office materials and equipment. The employee is frequently required to drive from location to location for periods of up to 1 hour, occasionally required to drive for periods of up to 3 hours or more.
- Be capable of visual and auditory scanning of the work environment to monitor the facility to ensure safe and effective office operations.
* “Medium” physical work refers to the ability to exert or lift up to 30 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to maneuver objects.
