As our company continues to grow We are seeking a per diem/part time Care Manager/social worker/ Aging Life Care Expert. Passionate about working with seniors, the chronically ill and disabled population.

As case load grows hours can increase very quickly. This position requires the flexibility of spending time in the office, working remotely and traveling locally.

Responsibilities:

Answers for Elders is a Care Management agency with expertise in Aging. We serve elderly members of the community in need of medical, psycho-social, emotional and financial care management.

The Social Worker’s role includes but is not limited to the following:

• Initiating, coordinating and communicating care plans for elderly clients.

• Conducting psycho-social evaluations, administering tests to assess cognitive abilities and mood.

• Understanding Medicare, Medicaid, Mass Health Planning and Long-Term Care policies.

• Maintaining contact with client responsible parties: family, physicians, attorneys and agencies.

• Works closely with in-house team for administrative support.

• In-depth documentation of all client interactions and outcomes.

• Negotiating with agencies for optimum service pricing based on client finances.

• Traveling throughout Plymouth, Barnstable, Bristol & Norfolk Counties for home visits and

appointments.

• Advocating for clients based on individual medical, financial and social needs.

Qualifications:

• LSW Required

• Experience in elder care

• Proficient knowledge of computer programs

• Excellent written and oral communication skills

• Reliable transportation

• Must have strong work ethic

• Must be looking for long term position

• Must be reliable and dependable

• self motivated, able to work independently

• is motivated by care & Commitment to seniors

Case Management and Elder Care skills

Socials Services skills

Knowledge of Medical terminology

Ability to work effectively in a team

Experience in geriatric care is a plus

Bachelor's degree in Social Work, Psychology, or related field
