Care Delivery Manager (RN) – $64/Hour | Full-Time | Flushing, Queens, NY

Schedule: Monday–Friday | 8:30 AM – 5:00 PM (1-hour lunch)
Pay Rate: $64/hour (W2)

A respected NYC-based PACE healthcare organization is seeking a Care Delivery Manager (Registered Nurse) to oversee clinical care coordination and interdisciplinary team operations at its Flushing, Queens location. This is a high-impact leadership opportunity to guide care teams, improve patient outcomes, and ensure compliance across managed long-term care services.

Care Manager (RN) Key Responsibilities:

• Lead interdisciplinary care team (IDT) meetings and manage participant care plans
• Supervise and support clinical nursing staff, ensuring optimal staffing and care delivery
• Monitor compliance with state/federal regulations, PACE standards, and documentation protocols
• Oversee incident investigations, care transitions, service authorizations, and quality reviews
• Coordinate long-term care placements and collaborate with providers across the continuum
• Evaluate nursing performance, provide coaching, conduct interviews, and assist in staff training
• Serve as a liaison across departments to promote efficient service delivery and participant satisfaction

Care Manager (RN) Required Qualifications:

• Current NYS Registered Nurse license (active and unrestricted)
• BSN required (or at least 5+ years of healthcare experience)
• Minimum 2+ years of clinical management experience in home care, long-term care, or acute settings
• MUST HAVE managed long-term care experience
• Prior experience in PACE, CHHA, LHCSA, or diagnostic/treatment centers preferred
• Infection control coursework (NYSED-approved); Case Management Certification preferred within 2 years
• Bilingual or conversational in Korean preferred
