JOB SUMMARY: To assess clinical needs of parents, caregivers, and children served by JFCS Child Safety Nassau County FAST Services, JFCS Nassau County STEPS services and Nassau County Dependency services. Provide individual and family therapy based on the assessed needs of the individual and/or family. Assess all child safety concerns and share information with the assigned Case Manager. For Nassau County TEAMS cases, assignment is at the time of removal by the Department of Children and Families. Determine needs for immediate services and make recommendations for ongoing services and parent/child visitation. Position is funded by Family Support Services so as contractual changes are made, adjustments may be made in job description and expectations.
Minimum Qualifications:
Education: Masters degree in Social Work , Mental Health Counseling, Marriage and Family Therapy or related field from an accredited college or university.
Experience: 2 years experience counseling children and families preferred.
Knowledge and skills in: Identification of abused and neglected children and adults; child development, individual and family functioning; working with hard to reach or resistant individuals and families; collaborating with other disciplines and services; skills in group dynamics and leading group discussions.
Licensure, Certification, and/or Registration: Licensed or license-eligible in the State of Florida (LMHC, LCSW or LMFT)
Primary Job Functions:
Clinical Work:
- Keeps records in compliance with all funding streams, including Medicaid.
- Works with all staff involved to ensure services are delivered in support of the family's case plan
- Participates in case staffings to discuss the individual/family's progress or response to therapy.
- Communicates regularly and is responsive (via email, phone or in person) with FAST FSCs, FAST Supervisors, DCF CPIs or Dependency FSCs and Clinical Supervisor about case updates
- Participates in consultations with FAST Supervisors, Case Managers, and Protective Investigators concerning individual cases as needed.
- Attends trainings, in service sessions, staff meetings and clinical supervision as required.
- Assists in Quality Improvement issues as requested or required.
- Complete other duties as assigned.
FAST/STEPS and Dependency cases:
- Provides in person and telehealth individual and family therapy to Nassau County FAST/STEPS and Dependency families, services are mainly provided in-home or school based, but can be community based if the need arises.
- Develop and maintain therapeutic relationship with families and children
- Develops, maintains and completes appropriate clinical records to include biopsychosocials, treatment plans, treatment plan reviews, addendums, progress notes, discharge summaries and monthly summaries, all within a timely manner and in accordance with record management policy.
TEAMS:
- Provides an immediate response (within 48 (business) hours of the shelter hearing) to the placement home to assess the child(ren) and caregiver at removal for immediate needs and support.
- Complete a minimum of 4-6 sessions with the family. Services are provided in the field, usually in the caregiver’s home.

b) Completes and updates treatment plans as required
c) Documents to reflect all services provided
d) Makes recommendations regarding ongoing services for the children.
- Meets with the parents to conduct assessments and make recommendations for ongoing services.
- Most of the work for this position requires being in the field. This will include home visits, school visits, and visits in the removal home with the parents.
- Coordinates and leads a multi-disciplinary staffing with the assigned case management team within 14 days of the shelter hearing.
- Enters documentation in Florida Safe Families Network (FSFN) and eCR for client records.
- Participates in weekly TEAM meetings.
- Works with all community partners involved to ensure services are delivered in support of the family's case plan
Participates in staffing with the QM Coordinator for data collection as needed.
