Job Description

Key Responsibilities

Patient Care: Administer medications, perform wound care, and assist with daily living activities such as bathing, dressing, and grooming.
Health Monitoring: Regularly check and document patients' vital signs, health status, and progress, reporting any changes to the supervising nurse or physician.
Care Coordination: Collaborate with other healthcare professionals, including RNs, physicians, and therapists, to develop and implement individualized care plans.
Patient Education: Educate patients and their families about health management, medication administration, and self-care techniques to promote independence and well-being.
Documentation: Maintain accurate and up-to-date patient records, including care provided, observations, and any changes in the patient's condition.
