Job Details

We are hiring a Registered Nurse (RN) to perform clinical assessments, 90-day client checkups, reassessments, and PCA training classes. This is not a direct care or routine in-home nursing role. Instead, the RN will oversee client health evaluations, maintain compliance, and coordinate with families and care teams to ensure continuity and quality of care.

Key Responsibilities:

• Conduct initial clinical assessments for new clients
• Complete 90-day client checkups to monitor condition and ensure care plans remain appropriate
• Perform reassessments when changes in condition or services occur
• Collaborate with caregivers, family members, and healthcare providers to coordinate care
• Provide documentation and support for plan of care development and updates
• Lead and/or assist with PCA training classes as needed by the agency
• Ensure clinical records and documentation meet agency and state compliance standards
• Serve as a clinical resource and support quality assurance efforts

Requirements:

• Active Registered Nurse (RN) license in the state of Virginia (unrestricted)
• Minimum of 1 year of nursing experience (home health or adult care experience preferred)
• Familiarity with Medicaid home care policies and reassessment protocols
• Comfortable conducting in-person client visits (assessments/checkups only, not routine care)
• Strong documentation, time management, and communication skills
• Valid driver’s license and reliable transportation for local travel
• Experience with or willingness to teach PCA training a plus
